New Patient Forms
If this is your first visit to Sie Eyecare, please complete our required New Patient Health History Form before your appointment. You’ll need Adobe Reader® to download and complete the form.
Download and print the form using the link below, fill in the required information, and either fax your completed form to our office at 704.548.0077 or bring it with you to your appointment.

Insurance & Payment Information
Understanding your benefits shouldn’t feel harder than taking care of your eyes. Sie Eyecare works with a variety of vision and medical insurance plans and offers payment options to help make your care easier to manage.
Because benefits and coverage can vary, we encourage you to learn more about the plans and payment options we currently accept or contact our team with questions about your individual coverage.
Cancellation & No-Show Policy
We understand that sometimes your schedule changes. If you need to cancel or reschedule your appointment, please let us know at least 24 hours beforehand.
Appointments canceled or rescheduled with less than 24 hours’ notice are subject to a $35 fee. The same $35 fee applies to missed appointments.
